We are committed to providing the highest level of service and professionalism to our community. Please read the following criteria for volunteers of Fight For Hearts, and sign below (by checking the box). I understand that safety of clients is the primary goal, and all programs/exercises must be executed with safety in mind. I will conduct myself in a professional manner at all times, including neat appearance and professional attire while volunteering for Fight For Hearts. I will also maintain professional boundaries and respect clients privacy and confidentiality. I understand that a background check and drug test may be required prior to start of volunteering. I understand that depending on my volunteer role and the program, Fight For Hearts may need to perform checks on all, or some of the following: Driving record, Credit Report, Employment/Volunteer verification, Education diploma verification, Personal References. I understand that Fight For Hearts reserves the right, at their sole discretion, to dismiss me from volunteer duties at any time, without cause or justification.
